Congratulations! You are admitted to Carolinas College of Health Sciences. Getting admitted to our college is no easy feat. Did you
realize that you are a member of a select group of students whose academic background was competitive enough to be chosen for admission?
The forms below are arranged by program or by topic to allow you to easily identify the necessary forms and information required for completion prior to starting.
Here are a few helpful tips for you to consider before enrolling at Carolinas College of Health Sciences.
- Students who are admitted to Nursing, Radiologic Technology, Radiation Therapy, Computed Tomography and Emergency Medical Sciences are required to have CPR(BCLS Level) certification before beginning classes. The Community Training Center

- Every student admitted to Carolinas College of Health Sciences is required to submit to a criminal background check by submitting the Student Information and Release
Authorization form (see below) and to complete a health assessment.
- New Student Orientation is an essential component to your success at Carolinas College of Health Sciences. For your convenience, we have linked a schedule of
events for this mandatory program.
- A uniform sheet is provided to assist you in ordering the necessary uniforms you need for your clinical work.
- If you have AP or CLEP credit, you may want to check out the Advanced Standing Credit link for more information about how those credits will transfer to our
college.
